Sid Adelman's Answer: The Business Intelligence Roadmap by Larissa T. Moss and Shaku Atre, Chapter 4, discusses gathering business requirements and The Data Warehouse Lifecycle Toolkit by Ralph Kimball, Laura Reeves, Margy Ross and Warren Thornthwaite has a great chapter by Margy Ross on the same topic.

Steve Hoberman's Answer: There are three templates I populate often when building a new system. The Family Tree documents a data elements lineage, the Grain Matrix is used exclusively for reporting and captures the levels of detail each fact should be viewed and the Data Quality Validation Template helps me analyze how the actual data compares to the meta data.
